What is the Transformation Suite?
The Transformation Suite is for systems that have stabilised performance and are ready to make the deeper structural changes needed for long-term sustainability. It brings together housing, health, social care, and voluntary sector partners around a shared model of integrated, preventative care.
This is not incremental improvement — it is a fundamental redesign of how care is organised, funded, and delivered. TMPM provides the strategic leadership, facilitation, and programme management to make complex, multi-partner transformation achievable and sustainable.
